This position will be mobile, in the Travis, Williamson, Bell, Hayes, and Bastrop Counties; patients and families may be referred to the Care Consultant directly upon diagnosis or medical appointments. Providing consultations and support, education, and resources, the Care Consultant ensures that each individual and their care partner receives a thorough understanding of Alzheimer’s/dementia supports and services available while also ensuring that related medical concerns are considered.
