Community Health Navigator - Austin Asian Community Health Initiative

Austin Asian Community Health Initiative is seeking a full-time (40 hours/week), bi-lingual Community Health Navigator (CHN) who speaks Arabic for its CHN Program. The goal of the CHN program is to provide community health system navigation services for Asian American communities; the incoming CHN will have a particular focus on serving Asian Arabic speaking communities. The CHN will report to the Program Manager and will be responsible for helping economically, linguistically and socially isolated community members and their families navigate and access healthcare and community services. They will help them identify and overcome barriers to care, provide health education, and connect them to resources. These services will promote self-management of health and wellness through the clinical–community links that enable culturally and contextually relevant care.

Job Duties include:

  • Reducing barriers that keep clinic patients from accessing healthcare by identifying patient needs and provide information and referrals
  • Offering health care system navigation services to the target population by facilitating enrollment in ACA (Obamacare) healthcare coverage, Medicaid, Medicare, Medical Access Program, or other healthcare options
  • Connecting patients with health literacy resources and help them make informed healthcare decisions
  • Providing advocacy support for issues such as patient rights, matters of privacy, confidentiality or informed consent, patient representation, awareness building, support and education of patients
  • Working with the Program Manager to develop a plan for targeted outreach, and potentially new strategies to engage clients
  • Building close relationships with community and social service organizations to identify community needs, planning and conducting community outreach and engagement to bring all stakeholders together
  • Demonstrating empathy, compassion and an understanding of the cultural and socioeconomic backgrounds to establish effective, trusting working relationships with patients and family caregivers
  • Managing and tracking a large amount of information, such as patient profiles, appointments and interactions in a systematic and organized manner
  • Meeting with the Program Manager regularly to ensure effective implementation of the program and track progress
  • Maintaining a knowledge base of available resources, both in-house and external
  • Maintaining patient privacy in compliance with HIPAA federal guidelines and AARC Nonprofit policies


  • Bilingual English & Arabic proficiency required; must be proficient in reading, writing and speaking in both languages
  • Must be able to effectively communicate (written and oral) and work well with a variety of stakeholders from different cultures
  • Minimum high school diploma or its equivalent required, Bachelor’s degree in social sciences, public health, nursing, social work or other related discipline preferred; experience may be substituted for education
  • Proficiency in Microsoft Office suite including Word, Excel, Outlook, and PowerPoint required
  • Must have drivers license and regular access to a car
  • Must have flexible work schedule to meet community needs, including availability some evenings and weekends
  • Must be willing to undergo a criminal background check
  • Must be authorized to work in the United States

Note: This position is a temporary full time position with an expected end date of September 30th, 2020 with a potential for extension.

Salary: $18.00 /hour
